What is happening with my form?

Hello!

I'm buildig a form, and I'm having a strange behavior. Please check:

As you can see there are several issues:
- Filds are not align. (I'd set the label width = 200 in each field)
- Tooltip not working
- Tooltip text is visible in the format field :: tooltip
- the lable is aligned in the bottom of the textArea

It is something wrong that I'm doing, or is it missing something?

Please set "Load Chronoforms CSS/JS Files?" to Yes in the form General tab.
