need quote on custom form

I have the form made but i cant seem to fix the glitches.

I need the form to be filled out then emailed and printed.

It can be emaild and print, but in Firefox,, it wont print the second page

The email wont send all of the numbers in the boxes.

I made the form in Dreamweaver CS3 and can send you the form
