Document woes

In the forms wizard, is it possible to make something similar to the document in the attached image, or would I have to do it in HTML by hand?

Hi bageton,

You could create the fields in the wizard - but then you'd need to edit the html by hand to get the layout you want.

You'd also need to decide if you wanted to input multiple line entries or just one at a time.

the best is to get this created as HTML then used in Chronoforms!
