Activating ChronoContact 3.1 Module Breaks Page

Module: mod_chronocontact_V3.1_RC4.11.zip
Component: ChronoContact 3.0 STABLE

Using ChronoContact the component has always been a dream, but I've never had to put a form in a module until today. I had no trouble installing the v3.1 module, creating a new form, and customizing the module's settings and positions. The form is enabled and green-checkmarked. The problem is, the second I enable the module on a page and reload, I get a solid white screen with no error at all. If I turn the module back off, the page reloads fine.

Any ideas?

There were some quite big code changes in RC4.11, you need to use the RC4.11 module with the ChronoForms RC 4.11 release.
