HELP !

Hi everybody !
I have 2 forms following to input data to database .
the first form is OK.
the second have a field(mahuyen) that GET data from a field(mahuyen) of the first form.
When i click Sunmit ( button ADD) , others field are saved OK, but the field mahuyen is blank.
Where is a error.
Help ME !
