Registration Forms with 6 Tabbed

Good Day!

I am looking for registration form with 6 tabs and then the 7th tab will show the complete registration form which can be print, email by user as well as received by administrator automatically as an html according to the PDF design.

received as a pdf is a plus..

I need this urgently using Joomla 1.5
