OK, I was trying to used the default form template. I created a form using the Wizard, then Transformed it. When I tried using it on the site, no data was included in the generated email or stored to the database. The debug wasn't showing that an email had been sent, but I was receiving an empty one. This was consistent in IE8. Firefox 3 was repeatedly giving me an error message, telling me to enter the data into the form again.
Just to try things out, I Transformed it again to the included "theme1" form template and tried it again. After submitting a form, I received an email populated with data and the database also held a populated entry. Then I Transformed the form back to the "default" theme and tried submitting it again and received a populated email and database entry.
Now both IE and Firefox are submitting the email and database entries correctly.

Hi,
Thanks for posting about the issue but nothing irregular here, please try to create a new form in the wizard and see if it gets the same problem ?

Max
