Form looks to send, but no data in email

Here's a link to my form I just created. I populated every field so that I would make sure that something would come through.

Hi swopeOffice,

The field name of Marital status includes a space name="Marital Status". This is technically illegal html and breaks the form code - replace it (and the id) with say Marital_Status and all should be well. (And please check the other fields at the same time - field names can have a-z,A-Z,0-9 and underscore, anything else may cause a problem.)

Hi, some of your fields names have spaces inside them, this is wrong and will not work fine, 2ndly, your checkboxes group should have 1 name with [], like bills[]
