Just found a few more things you may want to have a look at with the RC4.11 release...
When you use the 4.11 plugin and the 4.11 Module together on the same page (A form in the Module position and a different form in the content item via the plugin) than the Module will load the same form than the content item, even a different form is specified.
When I switch back to the V0.90 Plug-in than it loads again correct.
Also, when you use the V411 Module, it adds a libe break to the form, which goes again away when you switch back to the previous Module release ...
Just some things to look at ...

Thank you, yes, I have found this while doing a client's form and fixed it, so it will be ready in the next RC!🙂
