Error when I try to use a form in an article

Sorry if this is elementary, I am new to this.
I have created a form I want to use, but when I want to put it in an article I get this error

Kinda lost, any help would be great.

Hi David,

Do you have V3.1 RC4 ? if so then the latest module and mambot are not yet compatible with it!

I will add new ones today!
