hello, i want to create a tracking information form and when someone visits my website and search his package it will show the delivery status like the dhl or ups delivery

I'm sorry, the site here is getting busier and it really isn't practical for us to tell you how to code a commercial form from scratch. You will need to find someone who know enough about PHP and MySQL to write the code for you.
