What am I doing wrong?

Okay I got my form working and it looks like it is saving a record to the table in the db, it's not showing all the fields from my form, it only shows the timestamp, uid, ipaddress, cf_user_id, also the validation doesn't seem to be working, I only had 2 fields for validation, a phone # field to make sure that only #'s get input and a validation on the checkbox to make sure they agree to the terms and conditions. when you try to view the record, do you see ONLY those fields you mentioned or you see your form fields too but empty ?

regarding the validation, I see you have a beautiful validation which is working fine but if you are trying to use Chronoforms validation with this one then some conflict may occur!

If you only have the standard ChronoForms fields being saved maybe you didn't check the boxes for the other fields when you created the table??
