I'm sorry but this is a piece of work that would take some time and I just don't have the time at the moment. It's possible that I will be able to write a tutorial for you in the next week or two.

I think that the first step is for you to add these fields in CB so that they are in the jos_comprofiler table, ChronoForms will then pick them up automatically.
