Hi yatoula,
It looks OK in FireFox and Chrome but not in IE7. There must be some CSS conflict between the form css and the template css. You'll just need to dig into the css files and work out where the problem is. I don't see anything obvious but the problem seems to be with the width or float attributes of the label fields.
