The question is: is the a sh4004SEF Plugin for Chrono Forms 3.0
And that for Joomla 1.5

There's an alpha version on my site here - there are no clear docs about 1.5 plugins for sh404SEF so I haven't done much more than the most basic coding.

Sorry, that is for J 1.0, not 1.5.
And I am not using legacy mode.

Click on the dowload link and you'll find versions for 1.0 and 1.5

Yeah, but that is not for chronoforms specifically...
Then i would first have to alter all these xxxxxx files and such.

I had 2 forms in my menu's. After copying the plugin to my server, both got the sef url index.html.
But I could change them in the "View/Edit SEF Urls".

So it works, but is a bit buggy.

Yes, I make no claim for it being particularly good, it's just a port of the Joomla 1.0 version into the new sh404SEF wrapper. If/when Sumisha posts some docs for 1.5 I'm happy to look again.
