Hide fields in FORMS

I have a form that will have two radio buttons ...
By selecting one of two options, display your form below ...
Question:

How do to hide in the "email template" the fields of the form that has not been selected?

you need to use PHP in the email template to check the radio selected and hide/show some part of the template!

show me your email template code, if its more than 5 lines then please write a simplified version here so I can just write how to do it with the simple code!
