captcha image not showing

I have read through the posts and changed the permissions of the chrono_verification.php to 755
1) I still cannot see the captcha image on my form
2) When I try to access the URL http://www.ashakiran.com/asha/components/com_chronocomments/chrono_verification.php I get a 404 component not found.

Could someone suggest what I am missing.

Hi, this is the link to the form. The image does not show up but if you fill the fields and try to submit the form then it says "You have entered an incorrect verification code at the bottom of the form.". http://www.ashakiran.com/asha/rsvp.html

Max, I read from your other posts....I was not putting the {imageverification} in the form code. I assumed once it is turned on, it will automatically put it in....Thank you and great component.
