When I use a form in gantry in combination with the plugin the radio buttons stop working
Which plugin ?
The module that allows chrono forms in module position
Do you have the latest update installed ?
if yes then can you share a link to the form for testing ? you can send it using the Contact page if you want to keep it private

I think there was a JavaScript error on the first page but I could not check it carefully at the time, but now the form is gone
Did you try that page with the Joomla default template ? just to find out if the issue is only with Gantry or also on the default theme
I activated the form again, had to de activate since it wasnt working
Template used is JL Spark

Could you please test with the default Joomla "Cassiopeia" theme ? the module works fine without errors here, but on your page some JavaScript code is not defined on that page
Another "possible" solution is to add a "Custom" module to the same position, under the form module you have, inside that custom module make sure you enable Options > Prepare content
Then in the module content you can call the form using the ChronoForms8 content plugin, this "may" make a difference.
