Enable the form debug, does it show the correct email address under Data > location ? what does the debug show for the Email action ?

From the error image it shows the "From" email address is wrong or missing, did you enable the Email "From" behavior ? if yes then ensure it's configured correctly

Ok, I have just imported and tested it here, it's working fine, which may mean a problem with the Joomla global email settings "From address" on your website ?
