Please let me know how to fix issue, as there are lots of syntax showing on screen.

Does the form work in the admin area ? does it work when you click the Front view link in the forms manager ?

if the Front view link is not working then something on your web page is breaking the javascript code included by Chronoforms, please try to disable plugins which do some optimization or similar stuff as they may change the scripts code and this is breaking the page here

Yes created form in backend, but on front-end it is not working.

but did you test the view links I have shown ?
