Error, form not working anymore

Hello,

Form is not working anymore. After I made a recipients email "send to user" I get these error message.
It is not working anymore and not receiving any email.

it does not look like your form has any actions setup from the debug, can you post a screenshot for how the form is configured now ?

we need to check the "Settings" area and the "Actions"

Thanks for your response.
The strange thing is that it was working, I received the emails when testing but suddenly it stopped an the error was displayed.
