Subpage info into the mail result?

Hi, I use Chronoforms 6 on a website I made for a customer a good while ago.
Contact form appears on several pages/subpages.
Is it possible to insert a script or a fuction. So you can see what sub -page the inquiry comes from?

You can do that with PHP Action. Search on your favourite search engine.

Can you please point me in the right direction. What will be the best search terminology to get the correct result?
