Form doesn't show properly when used with mod_placehere

Question

I have experiencing this weird problem. I have a form that I placed in a module using mod_placehere for J!1.5. The thing is it does not load the form properly, the ajax doesn't load, the CSS does not load either, i don't know why. Below are screenshots. I also place a screenshot of the thing in firebug that I use together with my FF3. Please check it out. Thanks!

Here is the screen shot if show via the using menu > link type > chronocontact: (Notice that even tooltip properly appears)

Here is when i load the form in the right module using the plugin/mambot mod_placehere: (everything is messed up. i am sure that i followed all the instructions properly).

Note: Chronoforms for J!1.5 with Plugins/Mambot installed and configured properly.
