I need a form that will act a signup page and add member to the Joomla database, then offer special content.

Process:
Users will be given a private URL, go to the form, signup and then get directed to a content page that offers a free download.
Upon submit, if possible, have a thank you email sent to the users.

I want to create several forms with the same functionality, but would like to ID them so I will know where the members came form.
I assume you can track in CF if its a separate form.
