Calendar field not showing

I have a calendar field configured in my backend, but when opening the form as a user (frontend), this calendarfield is not showing..

I don't see any javascript errors on the page and in the html the field is nowhere to be found..
Any recommendations?

does it show when you test the form in the admin area ?

Do you have the latest v6 update installed ?
