Checkbox not working

I trayed to but checkboxes in my form, but the checkboxes are not shown in front end. I have no idea what I'm dorng wrong.

Can you help me with this issue?

I have already replied to your email about this. The problem is some setting in your template which is over-writing the ChronoForms CSS and setting the checkbox width to 0px.
