CF6 Wordpress SQL tables problem

Hello,
I have download chronoforms6.0.19 from this site, but I have an sql problem,
when I open tab "chronoform 6" I have these errors:

I have try to make a basic form with alias name "test-form"public and published
but if i put in my page
[Chronoforms6 chronoform="test-form"]
nothing is displayed

I use WordPress 4.9.6 with template Themify Ultra

Have you tried to recreate the form? I guess the issue may be also with the Themify. Do you have a Themify Updater plugin installed?
