Form name can NOT be empty

Hi
I am using Chronoforms5 with joomla 3.7
I have 3 languages in my website English and Dutch and Arabic

I have one Englis Chronoforms5 works fine but when I added locale for arabic language I get this error "Form name can NOT be empty"
when I am trying to dislplay the form

any body can help please?

I will tell what I did to make an Arabic form
According to this tutorial

I created new Locale and translate the labels names
Is that true or I have to do something else

Hi Bothman,

That is all that you should need to do - it appears that there is a problem with the SEF URLs somewhere that is mis-directing the form URL. How do you have those set up?

Hi Bob,
Actually I don't have experince to build website with Joomla,
what I did is
installing this quick_start https://www.joomdev.com/products/templates/jd-boston-template and run it.
this quick_start is using Chronoforms5 (in Englis) and when I tried to add multilanguage to the website I hade this problem with chronoforms5
