URL has changed?

Hi, my Chronoforms5 form for contacting us is somehow broken. The menu item leads to an error page. See https://www.astridtimmers.nl/component/com_chronoforms5/Itemid,145/view,form/

Strange thing is that the url seems to be really strange, I have not seen these comma's before in any of my url's.

anyone knowing of this issue?

This is the default Joomla SEF url, maybe you need to check what changed with that ?

yes I did. The form actually works already for years, and all of a sudden, it is broken..

Hi boudewijn,

Please try the following:
Download the latest v5 update and install it, with OUT uninstalling the old one.
Try to delete the old menu item and create a new one.
