Making text field enable/visable based on checkbox pick not working

Hi

I've tried to make an "Other: Please Specify" textbox field appear after the "Other" checkbox field is checked. There are various instances of this on the form, but I can never get it to work no matter what I put in the events tab. I've tried to follow the topics submitted to the forum, but none of them seem to work. I've no idea what I'm doing wrong.

It does not look like you have any events added to your checkbox, please post a screenshot of the events section, and your web page has many js errors.
