Hi Grey, thank you so much for your instant response.

We need to 2 things
1) We dont want to show label on the front end but we want to show lebel in the email.
2) Now, in this form, we have differnt departments and each department have differnt doctors. But in the email,we just want the doctor and department which was selected by the user.

Hi Chandan,

You will need to use a "Switch" action.
