iOS/Chronoforms issues??

Not sure what is happening here. I am getting sporadic reports of issues.

http://shintani.ca/nationals-ready.html

Seems on desktop (Windows or Mac) this works. Seems on iOS devices (from what I've seen so far) it does not work.

The http://shintani.ca/nationals-ready.html is Chronoforms Form Menu option for my Form called "Ready".

Suggestions?
