chronoform plugin not working

Chronoform plugin show the form inside article and when i click on submit button, it show article with form without sending email and show submit button.
also i search for Relative URL filed in form general tab according below article and this field is not there

Which version of ChronoForms are you using?

Does the form work from the Test or View Forms links in the Forms Manager?
