how to make 'repeated' form using 'submit-type' button

Hi,
i have a simple drop-down form with 4 options. Can I use 'submit button' (or other way?) to keep adding repeated form?
the basic layout is
-Item Name
-Item color
-Item weight
-Item Total

i want to allow use to repeat this set of questions upon request. Please help.
Also, is there any special code needed to capture all data? I am using the 'email' option to capture all the data.

When i use the multiplier (to add more form entry), the dropdown event does not load properly.
Only the first form entry would load the dropdown properly.

do i need to modify the options or i need modify the 'form code'.

Please help.

when Weight = 10g, Unit shows 10~50
when Weight != 10g, Unit shows 1-5

this form works on the 1st form, the Unit wont work if i add additional form.
please help.
