For some reason our interface does not look like the one in the video and we still can't find the "Send a copy to my own email address" feature - please see attached screenshot. The live form is here btw: http://www.goldcoasttransit.org/support/submit-your-comments

IIRC correctly, the video is using the Simple Wizard in that case but the settings are essentially the same.

To send an email to the user add the name of the email input to the Dynamic To box on the Advanced tab of the action.

To make that email Conditional use an Event Switcher action to check if the email is needed.
