I just noticed that firefox is blocking the form submission on this page - due to https issues with Chronoforms:

One more thing. I have two forms on that page, one in the page body and one in the footer (all pages). I'm noticing that the one in the body shows the above mentioned security issue and the one in the footer just doesn't work at all - I hit submit, the page refreshes and nothing has happened.

Please check the livesite setting in the Site Configuration file, that should be empty. ChronoForms uses the site settings for http/https and a livesite setting can cause this error.
The second problem is probably with the form action URL. Please see this FAQ

Thanks Bob, the livesite setting in configuration.php is empty. Any other ideas?
Also, I have tested the form via the "frontend view" link on the forms manager and it does submit correctly. The FAQ you provided however, is written for Chronofroms 5. I can't find the "relative URL" setting in v4.
Update: I tried inserting my site URL with "https" into the livesite setting and that seems to have done away with the https issue in Firefox. Still looking for the "relative URL" setting as described above though.
Hi again, that FAQ page doesn't seem to apply to v4 at all. I tried the "hidden menu" solution but can't find the "HTML (Render Form) action" either...
Ok, I found where the relative URL setting is in v4 (https://www.chronoengine.com/faqs/56-cfv4/cfv4-other-faqs/2571-how-do-i-show-a-form-in-an-article.html) and set it to "No".
That seems to have worked; however, now the captcha function is broken and keeps saying "You have entered a wrong verification code!".

It is. I'll have to deal with that later. For now, I've disabled the captcha and now am getting another weird behavior: the form is submitting successfully, but is redirecting to a debug screen. If you'll submit the form on this page, you'll see what I mean:

There is another form on that page - in the footer - that is not working. I've set that relative URL to "No", but it's not working. Sheesh.
Apparently, having two forms on one page is a problem. I've removed the footer contact form from the home page as it would not work, but was working on all other pages.
So the only remaining question for the moment is: why is the form on the home page redirecting to a debug display on submit?
