Submit doesn't work on live site

I am trying to install a new form. It works fine in the Test Form mode. But when I try to execute from the live site nothing happens. I have several other forms that I developed a couple of years ago and they are working fine.

My domain is members.californiadar.org. I have validated the install.

Hi Karen,

The Test Mode is the Live site so that part is OK. How are you displaying your form and what exactly is the problem?
