I seem to be having a problem that others have posted on, however, I did not find the solution on the forum. My apologies if this is redundant. I am using V5 and things have worked well until recently. None of our forms can be submitted. They all error out on Captcha Validation. When looking into the forums I noticed someone had the same problem. When my form loads and I enter the Captcha code I get "You have entered a wrong verification code!" every time. When I turn on firebug and click on the image and then follow the link in firebug I am shown a different image. If I enter the firebug code I get a successful submission. Please let me know what to do and let me know if I need to provide you with additional info.

Are you using the ChronoForms Captcha? Do you have site Caching on? That would explain the problem. If you load the page from a cache then the ChronoForms Captcha will always fail as it checks for a code created when the form loads. You can use Google Captcha OK if you need to have caching enabled.
