I'm putting together a simple form and upon submitting, the thank you message appears, but not inside of the Joomla template or with any formatting at all.

When the form is placed inside of an article (http://www.newproductsconference.com/index.php?option=com_content&view=article&id=214), I get this error message upon submitting.
500 - View not found [name, type, prefix]: article, PanelDiscussion, contentView
I've done similar forms on a number of other sites and have never run into this before.

Hi Zach,
Please change the name of the Format radio buttons to say session_format. What is happening is that Joomla! is taking format=xx to be a templating setting.
