Unable to view forms

Normally I don't have any trouble with Chronoforms, but I wasn't able to get a form to display so I uninstalled Chronoforms.

After reinstallation of the latest Chronoforms_V5.0.10_Component, I immediately attempt to view a Demo form and all I see is:

Form name can NOT be empty!

Is the demo form published?

Yes the demo form is published and I have modified it in any way.
