Dropdown show/hide text not working

I want to show a text input based on what is selected in a dropdown.

Kindly help me check what i have done wrong. i have even set up the debugger mode

Hi m4smariaoni,

There are some basic problems with the site. Without the template as far as I can see none of the JavaScript files are loading.

With the template some are loading but there are a bunch of errors that need fixing :-(
