Create a simple Mulitple Upload form for images

I need a basic image upload form created which will allow uploads of up to 14 images along with a name, email, contact number, and order number

Only Name, and email need to be required fields.

This form will then either email the files direct to a specified email account, or better yet, email her a link to the individual files stored in the client name folder on her server.
