Hmm . . . you could do this with ChronoForms but actually 95% (or more) of the code would be custom so you would only be using CF to load the code and save the results, The rest would be a lot of images and a lot of JavaScript - just under 1000 lines in the example page.

Do you have a detailed specification? And all the images prepared?
