Form name can NOT be empty!

Hi all,
I'm new.
I try to set a simple Chrono Forms to my web site, But if I go to the test page shows me the error "Form name can NOT be empty!"

If I "test" I see everything correctly (see annex), if you look at the site I see nothing (http://www2.borghetto.it/index.php/it/a)
How come?

How are you trying to show the form on this page? If you are using a menu please check that you have added the form name on the Menu Item 'Options' tab.
