Unable to create table

I have problems with my form, I just tried to create a table for my form and retrieve that message, any help please?

What are the names of the inputs in your form - unfortunately the useful parts are missing from the screen-shot :-(

If you have an email template you can copy and paste that here as it should include all the names.

I don't see any names that will cause a problem.

Please go to your Site Admin > Global Configuration and set Error Reporting to System Default or to None. that should hide the Warnings and hopefully the Table creation will then work OK.
