I have created a second form for a customers website, but for some reason I can't seem to get a table associated with it.
I have created a table by selecting the form and choosing Create Table, but it is not showing as you can see on the attached picture. I have not had this problem before, I hope it is me being stupid.

I can't tell anything useful from the image. Is the drop-down empty? That would be a bit odd. What is the name of the table? Does the name show in the DB Read or DB Save action in the form settings?

The second form I created was the one called RequestQuote.
I created the form as a new form, then selected it and chose Create Table.
In my form I have "There are no DB tables connected to this form to configure." when viewing the DB tab.
Should I not have a drop down box as per the DayTrip form? I several test versions of the daytrip form at one point, all of which had their own table.
Now everytime I try and create a new form I can't seem to get a table and have it connected.

The table will only show in the drop-down list after you have selected it in a DB Save action (possibly also in a DB Read - I'm not sure about that).

It was all my doing, its been so long since I created a form I completely forgot you needed to add a DB Action in the Events tab!!!
I found it from your DB Save comment and Google, which then pointed me to your Video on how to do it.
