Forms Keeps Refreshing Error

There are five forms that have been created and all of them seem to refresh intermittently back to the form after submission, which is odd behavior. The Thank You message sometimes displays, and we tried creating a Redirect URL with the same result occurring.

Has anyone else experienced this issue? Seems odd that the forms have worked before and now they ALL do not behave as they should after submission.

An example of one of the forms:

Hi toad,

Pleas try setting Relative URL to No in the form General Tab. I think that the SEF URLs are preventing the form from submitting correctly. A bit more in this FAQ.
