How to fix this problem?

If there's a database connection problem then you'll need to check the server logs/settings to see exactly what is happening.

Where do you see the error message - it doesn't show in the image? If it in the page HTML?

I create this form with relations to another database, and the name of database has changes. Maybe that is the problem. So what is the solution?

If you have changed the name of the database then you will - at least - need to reconnect any DB actions.
