ChronoConnectivity - "Add" and "Remove" buttons not working, redirects to index.php when clicked.

I was able to use link ChronoConnectivity to Chronoform and the "edit" function is working fine on the frontend. But the "New" and "Remove" buttons are not working, it redirects to the homepage (index.php) instead.

How can I make these buttons work?
