i have tried to find clear tutorial for uploading files but,i could'nt,i will appreciate any help.i wanna attach scan file in my form.but the upload action not working😟

the configuration of my upload file action is:

Upload Path: C:\AppServ\www\yashouse\components\com_chronoforms\uploads\crm\
Max Size in KB:1000
Min Size in KB:0

as i test the form i cant see my attached file in the path i have set.
should i do sth in database?
